I had the same problem, and found the following arcitle in
microsoft.public.pocketpc from 2/10/2003:

Well, thanks to Michael Boone of Biohazard Software (Maker of Data on
the Run) the problem has been solved for me. He suggested that I run
regsvrce.exe and register aosync.dll with the os. I backed up my
toshiba e740 with pocketpc2002, installed regsvrce.exe, and put the
toshiba in the cradle. After active sync and the toshiba recognized
each other, I opened options, found Pocket Access with a checkbox,
filled it in. NOW it Works.

I downloaded RegSvr32.exe from
http://www.pocketc.net/release/Distribute/regsvrce.zip, loaded it on
the Pocket PC, ran it on "\Windows\Adosync.dll", pulled it out of the
cradle, enabled Pocket Access, and next time I put it in it picked
right up.
Looks like when I uninstalled a Demo program somewhere it unregsitered
the .dll, and broke Pocket Access.
